Staff: Community Service Input Form

Please take a few minutes to help the University gather information on community service activities being performed by staff in a professional and/or personal capacity.

Information from this survey is used in public materials, such as University media releases, Community Relations e-newsletters, and applications for recognition, such as the annual President’s Honor Roll for Community Service. This year it will also help to inform a 2018 Community and Economic Impact Report.
Please take a few moments to complete the form with information on activities you participated in during the 2017-2018 academic year and help us track and celebrate community service activities. If you complete the form between now and June 22, 2018, you will be entered to win a $100 gift certificate at POSH in downtown Scranton.
